PATIENT RESPONSIBILITIES

The right to access health care and the provision of preventive services.

Rights to free choice of doctor and healthcare provider.

The right to adequate, high-quality and safe health care.

The right to respect the patient's time.

The right to information and cooperation.

The right to make independent decisions about treatment.

The right to prevent and alleviate suffering.

The right to a second opinion.

The right to access medical documentation.

The right to privacy and protection of personal data.

The right to careful care and respect.

The right to preserve one's cultural identity.

The right to racial tolerance and to express one's beliefs.

The right to care with the least possible risk.

The right to provide services free from abuse, exploitation and discrimination.

The right to equal access and treatment in medical treatment.

The right to take into account a previously expressed will.

The right to address violations of patient rights.

The right to free assistance in exercising the patient's rights.

During illness, to adhere to received professional instructions and treatment plans, providing oral or written consent.

To provide the attending physician and other authorized healthcare professionals or collaborators with all necessary and truthful information regarding one's health condition that is known and relevant for ongoing healthcare, including details of current and past injuries and illnesses, their treatments, familial illnesses, any allergies, and medications being taken.

To inform healthcare professionals and collaborators promptly about sudden changes in health status that occur during treatment.

To be considerate and respectful of the privacy and other rights of fellow patients, healthcare professionals, and collaborators.

To adhere to published schedules, house rules, and prescribed organizational procedures of healthcare service providers.

To promptly notify the healthcare service provider of any absence from scheduled appointments or treatments.
